A new study will examine how e-cigarette use during pregnancy affects fetal lung immunity and increases infection susceptibility in newborns.

A comprehensive five-year research project has been announced to investigate the effects of prenatal exposure to e-cigarettes on fetal lung immunity. The study, funded with a grant of $2.4 million, aims to evaluate how various components and flavours of e-cigarettes impact the development of the respiratory immune system in fetuses.

The research addresses a significant gap in knowledge regarding the implications of e-cigarette use during pregnancy, particularly as these products are often considered less harmful than traditional combustible tobacco. Despite this perception, the potential effects on developing immune systems remain poorly understood.

Preliminary laboratory findings suggest that exposure to components found in e-cigarettes can disrupt innate immune pathways within the fetal lung. Changes observed during initial studies include alterations in viral sensing and signalling pathways associated with immune responses, such as type I interferon signalling and neutrophil activity. Notably, these effects appear to vary between male and female offspring, indicating differing susceptibility to respiratory infections post-birth.

The study will delve into how prenatal exposure influences important immune cells, including neutrophils, macrophages, natural killer cells, and alveolar epithelial cells. Researchers will employ advanced techniques such as spectral flow cytometry, confocal imaging, and single-cell RNA sequencing to characterise immune alterations resulting from e-cigarette exposure.

Furthermore, the project will utilise computational modelling alongside machine learning techniques to map affected gene networks and signalling pathways. Another objective of the research is to assess whether postnatal treatment with interferon beta can restore immune function and correct compromised genetic pathways in newborns previously exposed to e-cigarette components.

Investigators will also examine biological sex as a critical factor influencing fetal immune programming. Initial evidence suggests that male and female fetal organs respond differently to exposure during gestation, which may lead to varying respiratory health outcomes.

The findings from this research could significantly enhance understanding of how prenatal exposure to e-cigarettes influences immunity and increases the risk of respiratory infections in infants. Ultimately, the outcomes may assist in developing better counselling and prevention strategies for expectant mothers and inform future health interventions for children born to mothers who vape during pregnancy.
